Make an Appointment

Physician referral is not mandatory for patients with commercial or PPO insurance. For Managed Care/HMO patients, your Primary Care Physician must refer you to the clinic. Please fax or mail referrals to the Referral Coordinator. Referrals should include permission to perform skin testing and pulmonary function testing. Please be sure to bring any pertinent medical records and x-rays to your first appointment.

Please read what else to do Before Your Visit. You should also bring a list of medications you may be taking, or the medications themselves.

Payments

The initial comprehensive evaluation includes a detailed history and physical exam and, possibly, laboratory tests, as well as skin testing and spirometry. We will file your insurance, however, we do expect payment of your co-pay at the time of service. For your convenience, payment may be made with credit cards (VISA, MasterCard), cash, check or money order.
