Make an Appointment

Physician referral is not mandatory for patients with commercial or PPO insurance. For Managed Care/HMO patients, your Primary Care Physician must refer all patients to the clinic. Please fax or mail referrals to the Referral Coordinator. Referrals should include permission to perform skin testing and pulmonary function testing. Please be sure to bring any pertinent medical records and x-rays to your first appointment.
